1. Our Mission
The Brockport Area Health Foundation supports nonprofit organizations that address community health needs by promoting access to healthcare, preventative services, and resources that improve physical, emotional, and social well-being.
2. Funding Priorities

• Mental Health & Substance Use: Community-based mental health support and substance use disorder prevention.

• Healthy Women, Infants & Children: Maternal health, prenatal/postnatal care, child nutrition, and development.

• Chronic Disease Prevention & Management: Health education and disease management programs.

• Basic Needs: Food, hygiene, clothing, and support for low-income populations.

3. Eligibility Requirements
Applicants must:

• Be a 501(c)(3) nonprofit

• Serve the Brockport, NY area

• Show measurable impact on health

• Provide nonprofit status and financials

4. Grant Size and Duration

• Awards typically range from $1,000 to $15,000

• Projects should be completed within 12 months

• Multi-year funding may be considered for proven success
